Job summary

Indiana Treatment Centers in Mishawaka seeks a Care Coordinator to guide patients from admission through discharge in a collaborative, trauma-informed environment. The role involves coordinating intake, discharge planning, and aftercare with Admissions, Clinical, Medical, Recovery Support, and Utilization Review teams to remove barriers to care.

This position emphasizes patient-centered care, documentation accuracy, and coordinating services across the care continuum in an EMR environment.

Qualifications

• High school diploma or equivalent.
• Experience in behavioral health, substance use disorder treatment, healthcare coordination, case management, or related field.
• Strong organizational skills and ability to manage multiple priorities in a fast-paced environment.
• Excellent communication skills, both written and verbal.
• Comfort working in EMRs and clinical documentation systems.
• Associate’s or Bachelor’s degree in social work, human services, psychology, or related field (preferred).
• Experience in residential or inpatient SUD treatment settings (preferred).
• Familiarity with intake, discharge planning, or care coordination workflows (preferred).
• Knowledge of trauma-informed care and recovery-oriented systems of care (preferred).

Responsibilities

• Coordinate intake assessments, documentation, and handoffs.
• Partner with Admissions, Nursing, and Clinical teams to ensure timely intake processes.
• Ensure required consents, screenings, and documentation meet policy and regulatory standards.
• Serve as a welcoming, trauma-informed point of contact during admission.
• Coordinate discharge planning with clinical and medical teams.
• Arrange aftercare services, referrals, appointments, and follow-up care.
• Facilitate warm handoffs to outpatient providers, residential programs, or community resources.
• Support patients and families in understanding next steps and ongoing care plans.
• Monitor engagement, attendance, and care milestones; escalate as needed.
• Participate in treatment team meetings and collaborate with Utilization Review.
