Description

Responsible for providing hands on assistance to residents in a licensed assisted living facility, as needed, with daily living activities, to include health, personal care, lifestyles and care management. Ensure residents physical and psychological needs are cared for within the limits of their ability. Perform specific duties and services needed by the resident. Under the direction and supervision of Director of Assisted Living, and/or licensed nurse, perform all tasks as assigned.

Responsibilities and Duties
Resident Services:
  • Answer and respond immediately to resident calls.
  • Assist residents with activities of daily living including but not limited to showering, bathing and/or toilet needs including attending to residents who are incontinent.
  • Assist with resident activities as needed.
  • Assist with meals in the dining room.
  • Assist with cleaning outside of housekeeping hours as needed.
  • Assist with or do residents’ personal laundry and linens as needed.
  • Establish and maintain good relationship with residents and their families.
  • Ensure that all systems for providing and tracking resident care, specifically all forms in ECP, are completed in accordance with the residence policies and procedures.
  • Observe residents for emotional, behavioral, and physical changes in condition including, but not limited to any changes in mood, skin integrity, bowel and bladder status, food/fluid intake, speech, ambulation and transfer ability and promptly report any changes in resident condition to Resident Care Coordinator, Lead Resident Assistant, or RN.
  • Respond to medical emergencies calmly and promptly in accordance with established policies and procedures. Make emergency calls to 911.
  • Attend staff and resident care planning meetings and in-services as required.
  • Report any maintenance needs to ACC Property manager.
Medication Duties:

Assist with medication and/or treatment administration under the direction of facility RN.

Assist with medications as described in the medication administration policy.

Report resident change in condition or concerns regarding resident and/or resident’s medication to facility RN, Resident Care Coordinator, or Director of Assisted Living immediately.

Ensure resident receives medication and that all documentation is completed accurately.

Additional Duties:

Review any and all communication tools used in providing resident care on each shift.

Communicate residents’ needs with Resident Assistant staff at change of shift and as needed.

Comply with State of Wisconsin rules and regulations

Accurately record resident data on appropriate forms in accordance with established policies and procedures such as incident reports, notes, and task sign-off.

Inventory and keep track of supplies needed such as Ensure, med cups, PRN meds, gloves, etc. and inform Resident Care Coordinator and/or facility RN.

Answer telephones and return calls timely and in a professional manner.

Additional duties as assigned by Lead Resident Assistant, Resident Care Coordinator, facility RN, Director of Assisted Living.

Requirements
Knowledge and Qualifications:

High School Diploma required;

Must have at least one year experience as a nursing assistant and CBRF certified caregiver preferably experience with advanced age.

Completion of all required CBRF trainings upon hire and completion of 15 CEUs per year.

Proficient in English

Effective communication and interpersonal skills with the ability to work effectively as part of a team

Ability to attend and participate in departmental meetings, in-services and training as required

Availability to work nights, rotating weekends and holidays

Must have genuine interest in the needs of the advanced aging, mental health and other infirmities of age, understanding the problems they encounter and have patience and tact in servicing their needs.

Maintain knowledge of federal and state laws pertaining to assisted living (Chapter DHS 89)

Able to remember multiple tasks/assignments over long periods of time

Excellent oral and written communication skills;

Excellent customer service skills;

Knowledge of or ability to learn computer programs specific, including an Electronic Medication Administration Record system and a Nurse Call system;

Proficient in computer use and Microsoft Office applications;

Strong and demonstrated organizational skills;

Ability to occasionally work outside of regular work hours, as needed;

Ability to maintain resident confidentiality at all times.

Physical Qualifications:

Ability to lift/carry, push/ pull up to 50 pounds

Ability to walk and stand for prolonged periods of time, and reach, twist, sit, squat climb stairs, reach above shoulders and bend at knees
