Job summary

A healthcare solutions company is seeking a Quality Outreach Coordinator to assist high-risk members with closing care gaps. Responsibilities include member outreach, data entry, and supporting quality initiatives. Candidates should have a high school diploma and at least 2 years in a managed care setting. This is a remote position offering an 8-month contract with possibilities for extension.

Qualifications

  • 2 years in a managed care setting, medical office or facility setting with demonstration of medical administration duties.
  • No outbound phone experience, no case manager experience.

Responsibilities

  • Interact with high-risk members and/or providers to improve healthcare outcomes.
  • Support quality initiatives including outreach to noncompliant members.
  • Conduct seasonal call campaigns in coordination with local market efforts.
  • Enter data assessments and authorizations into the system.
  • Maintain integrity of Personal Health Information.

Summary

Performs duties to assist in activities related to closing open care gaps for high-risk members.

Key Responsibilities
  • Interacts with high-risk members and/or providers to improve healthcare outcomes and close care gaps such as increased preventive care compliance
  • Provides support for quality initiatives including outreach to members who are noncompliant in the designated outreach services
  • Conducts seasonal call campaigns, in coordination with local market efforts
  • Enters data assessments and authorizations into the system and maintain database as directed
  • Maintains integrity of Personal Health Information
  • Performs other duties as assigned
  • Complies with all policies and standards
Qualifications
  • High school diploma or equivalent
  • 2 years managed care setting, medical office or facility setting with demonstration of medical administration duties
  • Knowledge of medical terminology
