Job summary

PrimaryOne Health in Columbus, OH is seeking a Quality Improvement Specialist to advance patient care and clinical quality across our urban health center. You will lead and manage QI projects, collaborate with providers, nurses, informatics, and care teams to implement evidence-based improvement processes.

  • Experience with clinical quality measures (e.g. UDS, HEDIS, or internal scorecards).

Responsibilities

  • Lead and manage QI projects in alignment with the FQHC's strategic goals, compliance mandates, and clinical priorities.
  • Collaborate with medical providers, nursing, informatics, and care teams to implement evidence-based quality improvement processes.
  • Develop, support, track, and disseminate quality scorecards and dashboards; provide regular updates to leadership and clinical teams.
  • Ensure organizational quality efforts align with HRSA Clinical Quality Measures, UDS reporting, PCMH and FTCA compliance standards.
  • Coordinate and support preparation for OSVs, audits, and internal reviews with documentation and performance data.
  • Analyze health outcome data, identify trends and disparities, and recommend interventions to improve population health and clinical performance.
  • Facilitate root cause analyses, process mapping, and PDSA cycles as part of CQI methodology.
  • Educate and support staff on quality principles, performance improvement tools, and compliance standards.
  • Maintain familiarity with regulatory changes, quality reporting mandates, and clinical best practices to ensure ongoing readiness and compliance.
  • Participate in Quality, Risk Management, and Compliance Committee meetings, and contribute to performance improvement plans.
  • Moderate travel required.
  • Other duties and special projects as assigned.

PrimaryOne Health is currently seeking a Quality Improvement Specialist.

Essential Duties and Responsibilities
  • Lead and manage QI projects in alignment with the FQHC\'s strategic goals, compliance mandates, and clinical priorities.
  • Collaborate closely with medical providers, nursing, informatics, and care team members to identify, implement, and monitor evidence-based quality improvement processes.
  • Develop, support, track, and disseminate quality scorecards and dashboards by partnering with our Quality Analysts. Provide regular updates to leadership and clinical teams.
  • Ensure organizational quality efforts align with HRSA Clinical Quality Measures, Uniform Data System (UDS) reporting requirements, Patient Centered Medical Home (PCMH) and FTCA compliance standards.
  • Coordinate and support preparation for Operational Site Visits (OSVs), audits, and internal reviews by maintaining documentation, performance data, and improvement evidence.
  • Analyze health outcome data, identify trends and disparities, and recommend interventions to improve population health and clinical performance.
  • Facilitate root cause analyses, process mapping, and Plan-Do-Study-Act (PDSA) cycles as part of CQI methodology.
  • Educate and support staff on quality principles, performance improvement tools, and compliance standards.
  • Maintain familiarity with regulatory changes, quality reporting mandates, and clinical best practices to ensure ongoing readiness and compliance.
  • Participate in Quality, Risk Management, and Compliance Committee meetings, and contribute to performance improvement plans.

Successful candidates must have an active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Ohio. Minimum three years of clinical experience, preferably in a primary care or community health setting. Minimum three years\' experience in a quality improvement role, preferably in a FQHC or healthcare setting. Experience working with clinical quality measures (e.g. UDS, HEDIS, or internal scorecards).
