Hireology seeks a Medical Record Auditor to join our home health team in Wichita, KS. You will review patient records for accuracy, completeness, and regulatory compliance, and conduct audits of clinical documentation to support care plans.

Ideal candidates have experience in home health, healthcare documentation, and EMR systems, with strong attention to detail and the ability to work independently while collaborating with clinical staff.

Qualifications

• Strong knowledge of medical terminology and healthcare documentation.
• Familiarity with Medicare/Medicaid home health requirements.
• Excellent attention to detail and organizational skills.
• Ability to identify documentation issues and communicate them professionally.

Responsibilities

• Review patient medical records for accuracy, completeness, and compliance.
• Conduct routine and focused audits of clinical documentation.
• Ensure documentation supports the services provided and the patient's plan of care.
• Identify documentation deficiencies and communicate findings to appropriate staff.
• Monitor compliance with Medicare, Medicaid, and other applicable regulatory requirements.
• Assist with identifying trends and opportunities for improvement.
• Work with clinical leadership and staff to improve documentation quality.
• Maintain accurate audit records and prepare reports as needed.
• Assist with preparation for surveys, audits, and regulatory reviews.
• Maintain confidentiality of patient and employee information in accordance with HIPAA and company policies.

Are you an experienced healthcare professional with a strong eye for detail and a passion for quality? We are looking for a Medical Record Auditor to join our team and help ensure our patient records meet regulatory, clinical, and organizational standards.

Position: Medical Record Auditor

What You’ll Do:

• Review patient medical records for accuracy, completeness, and compliance.
• Conduct routine and focused audits of clinical documentation.
• Ensure documentation supports the services provided and the patient's plan of care.

What We’re Looking For:

• Previous experience in home health, healthcare documentation, medical records, or clinical auditing preferred.
• Strong knowledge of medical terminology and healthcare documentation.
• Familiarity with Medicare/Medicaid home health requirements preferred.
• Excellent attention to detail and organizational skills.
• Ability to identify documentation issues and communicate them professionally.
• Strong computer and electronic medical record skills.
• Ability to work independently while collaborating effectively with the clinical team.

Preferred Qualifications:

• RN, LPN, HIM, or other healthcare-related credential preferred.
• Experience with OASIS, Medicare Conditions of Participation, and home health documentation is a plus.
• Experience conducting medical record or quality assurance audits preferred.
