Job summary

VitalCore Health Strategies is seeking a Quality Assurance Registered Nurse (RN) for Baylor Women’s Correctional Institution in New Castle, Delaware, to join as a Full-Time Day Shift team member. The CQI nurse monitors patients from intake to the facility and ensures continuity of care, advocating for patient needs while coordinating with multiple systems in a correctional environment.

Qualifications

• Experience in healthcare setting prefer correctional or managed care environments.
• Strong communication and collaboration with multidisciplinary teams.
• Ability to monitor and respond to patient needs across care transitions.

Responsibilities

• Monitor patients from intake through release, ensuring continuity of care.
• Act as liaison between inpatient, community, and correctional health systems to advocate for patients.
• Oversee daily care management calls and site log, including infirmary placement, COVID monitoring, Behavioral Health patients, hunger strikes and crisis management.

Summary

The CQI nurse monitors patients from intake to the facility andensures continuity of care leading up to and following release into the community by advocating for patient needs while acting as a liaison between the multiple systems impacting the lives of offenders to provide a proactive and collaborative approach in serving the patient population in a correctional environment. The Care Management (CQI)- Registered Nurse oversees the daily care management case calls and site log, to include infirmary placement, COVID monitoring, Behavioral Health patients, hunger strikes, and crisis management.
