Job summary

Sunrise Community of Northeast Florida, Inc. is seeking a Direct Support Professional to help individuals with intellectual and developmental disabilities live as independently as possible.

You will coordinate and monitor each person's Program Plan and collaborate with a multidisciplinary team. Responsibilities include documenting progress, preparing plans, and assisting clients with access to Medicaid/Medicare benefits.

Qualifications

• Must possess a valid Driver's License
• Bachelor's Degree in Human Services required
• At least 1 year experience in IDD field
• Must pass pre-employment background and drug screenings
• Basic computer skills required

Responsibilities

• Oversees and coordinates the Program Plan of each individual and ensures it aligns with the individual's needs and desires
• Creates positive, collaborative communication with all disciplines and team members
• Ensures Program Plans and documentation are completed in accordance with all regulatory agencies
• Assists in monitoring and training of staff to ensure accurate implementation and documentation
• Responsible for assisting individuals to apply for third party benefits such as Medicaid and Medicare
• Performs all other duties as assigned

Qualifications:

• Must successfully complete the required pre-employment Background and Drug Screenings

• Must possess a Bachelor's Degree in the Human Services field
• Must have at least one year experience working in the field of intellectual/developmental disabilities
• Experience in writing program plans, case notes or other relevant documentation preferred

Essential Functions:

• Responsible for assisting individuals to apply for third party benefits such as Medicaid and Medicare
• Performs all other duties as assigned

This position will remain open until filled.
