Job summary

Alteas Health is seeking a Nurse Practitioner or Physician Assistant to provide specialty Pulmonology care across Skilled Nursing, Long-Term Care, and Post-Acute Care facilities in the Cincinnati area. Daily travel within Loveland to Norwood and Fairfield to Sharonville regions is required, with a Monday–Friday daytime schedule and no weekend on-call duties.

You will work collaboratively with physicians and care teams to diagnose, treat, and manage pulmonary conditions, prescribing medications

Qualifications

• Current, unrestricted Nurse Practitioner license in the applicable state(s) of practice.
• Current national certification appropriate to licensure.
• Current Basic Life Support (BLS) certification.
• Minimum six (6) months of Nurse Practitioner clinical experience.
• Valid driver's license with reliable transportation.
• Ability to travel daily throughout an assigned territory.
• Strong clinical judgment, communication, organizational, and critical thinking skills.
• Experience utilizing Electronic Medical Records (EMR).

Responsibilities

• Evaluate, diagnose, and manage acute, chronic, and episodic pulmonary conditions.
• Develop individualized treatment plans using evidence-based clinical practice.
• Order, interpret, and communicate laboratory studies, imaging, and other diagnostic testing.
• Prescribe medications and treatments within your scope of practice.
• Educate patients, families, and caregivers regarding diagnoses, treatment plans, medications, and ongoing care.
• Coordinate care with physicians, nursing staff, therapy, pharmacy, social services, and other healthcare professionals.
• Maintain accurate, timely, and compliant clinical documentation.
• Round at assigned Skilled Nursing, Long-Term Care, and Post-Acute Care facilities.
• Build strong relationships with facility leadership and clinical teams.
• Represent Alteas Health with professionalism, integrity, compassion, and clinical excellence.
