Job summary

Community Health Network is seeking a Nurse Practitioner or Physician Assistant to join the Pulmonary Hypertension team at Community Hospital North in Indianapolis, IN. The role covers both clinic and inpatient rounding, with a schedule of four 10-hour shifts per week, including in-office and virtual home visits.

Qualified candidates will have current licensure as an NP or PA in Indiana, experience in pulmonary hypertension, heart failure, or palliative care, and strong communication, critical

• Experience as a licensed Nurse Practitioner or Physician Assistant.
• Pulmonary Hypertension, Heart Failure, or Palliative Care experience is strongly preferred.
• Excellent communication, critical thinking, and planning skills.
• Must be hardworking, proactive and dedicated to high-quality patient care.
• Must be comfortable assessing patients independently.

Responsibilities

• Work alongside the Pulmonary Hypertension team at Community Hospital North in Indianapolis, IN.
• Clinic and inpatient hospital rounding in the Pulmonary Hypertension program.
• 4 x 10 hour shifts weekly, including in-office and virtual home visits.
