Qualifications

Responsibilities

  • Provide comprehensive HIV/AIDS care, monitor health, administer meds, manage symptoms.
  • Offer counseling to patients about fears and challenges related to HIV/AIDS.
  • Maintain wellness activities: immunizations, screening exams, follow-up labs.
  • Develop care plans with providers; coordinate interagency referrals.
  • Assess readiness for ART and long-term HAART management.
  • Administer medications per protocol, explain meds, obtain consent and document.
  • Follow infection control protocols to prevent transmission in clinic.
  • Coordinate with services to ensure coordinated care delivery.
  • Educate patients and communities about HIV transmission and prevention.
  • Handle crises and after-hours calls; stay updated with HIV treatment advances.

Position Highlights:
  • 1.0 FTE (40 hours per week).
  • $39.69-$48.62/hour DOE with the ability to go higher for highly experienced candidates.
  • $7,000.00 Hiring/Retention Bonus Structure:
    • At Hire: $2,100.00 .
    • At 180 days (6 months): $2,800.00.
    • At 12 months: $2,100.00.
    • Bonus will be prorated based on Full-Time Equivalency (FTE).

What You’ll Do:
  • Provides comprehensive specialized care and support to patients living with HIV/AIDS by monitoring and assessing patients’ health, administering medications, and managing symptoms related to HIV/AIDS.
  • Provides emotional support and counseling to patients, addressing their fears, concerns, and challenges related to living with HIV/AIDS.
  • Maintains wellness activities for patients including immunizations, screening exams, and follow-up lab work. Offers guidance on safe practices, including safe sex and needle exchange programs to prevent the spread of HIV/AIDS.
  • Develops care plans with the patient in collaboration with the provider, based on medical conditions, health goals, and behaviors. Collaborates with other healthcare providers in developing plan of care. Coordinates and plans interagency referrals related to patient care as directed by the provider.
  • Provides assessment, education, and evaluation of patients’ readiness to begin Antiretroviral Therapy (ART). Provides ongoing long-term management for patients receiving HAART therapy.
  • Administers medication to patients as directed by the provider, protocol, and standing orders. Identifies the purpose, side effects, and onset of action for each class of drug being given to the patient. Correctly explains the medication to the patient/guardian and obtains consent. Regularly monitors patients' viral loads, CD4 counts, and overall health to track progress and adjust treatment plans as necessary.
  • Follow strict infection control protocols to prevent the transmission of HIV/AIDs within the clinic setting.
  • Maintains appropriate ongoing assessment, interventions, education, and referrals as identified in the care plan.
  • Maintains regular contact and works collaboratively with other services and professionals involved in delivering services to the client to ensure coordinated service delivery.
  • Provides HIV counseling and coordination of testing for high-risk individuals.
  • Completes chart notes per standards in EPIC and Provide systems. Updates client data in the Electronic Medical Record (EMR) systems to document health assessment and health history. Records care plans and education provided. Documents assessments, actions, and medications in medical records.
  • Provides education to a wide variety of audiences, including community members and healthcare professionals regarding HIV, including transmission and risk reduction. Collaborates with community outreach programs to raise awareness about HIV/AIDS, promote testing, and provide support to affected individuals and their families.
  • Advocates for patients' rights and access to health care services, ensuring they receive appropriate medical and social support.
  • Crisis management: handles medical emergencies and crises effectively, providing immediate care and coordinating with the health care team as needed. This includes taking calls after hours.
  • Continuing education: stay updated with the latest advancements in HIV/AIDS treatments, medications, and healthcare practices through continued education and training.
  • Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), Department of Health (DOH), and YVFWC requirements.
  • Perform other duties as assigned.
Qualifications:
  • Associate’s Degree in Nursing.
  • Bachelor's Degree in Nursing preferred.
  • One year of RN experience.
  • Registered Nurse license for the state of practice.
  • If RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply:
    • Must submit proof of license in the state of residence.
    • Must submit proof of completion for the required suicide prevention training during the 1st week of onboarding.
    • If relocating to Washington State, must obtain WA RN license within 6 months of hire.
    • Must update primary state of residence within 60 days (applies to anyone relocating to Washington State)
    • Military Nurses are not required to obtain Nurse Licensure Compact (NLC)
  • Basic Cardiac Life Support (BLS) Certification required upon hire.
  • Advanced Cardiac Life Support (ACLS) Certification preferred.
  • Spanish bilingual proficiency is preferred at a level 10.
  • This position is eligible for Bilingual Differential Compensation when an ALTA score of 10 is achieved.
  • Knowledge of established professional nursing concepts and all nursing procedures.
  • Knowledge of the uses and effects of medications.
