Job summary

State of Maine's Department of Health and Human Services invites applications for a Public Health Nurse Navigator in Augusta. You will prioritize and assign referrals, coordinate care with hospitals, providers, schools, and agencies, and ensure completeness of referrals.

This role supports Maine CDC initiatives and requires RN licensure with strong communication skills. Partial telework is allowed with management approval, and the role emphasizes collaboration with the community to promote PHN

Qualifications

• Graduation from an accredited school of nursing.
• Licensed as a Registered Nurse (RN) in the state.
• Experience with electronic medical records (preferred).
• Ability to communicate effectively orally and in writing; work well independently.

Responsibilities

• Prioritize, review and assign public health referrals.
• Monitor receipt of new referrals and determine services required.
• Coordinate client care with hospitals, providers, schools, and agencies.
• Process referrals via CradleMe referral system.
• Assist nursing management on projects and represent PHN in the community.
