Job summary

The PHN III will maintain a caseload, perform investigations, and collaborate with partners to ensure program compliance, reporting, and quality improvement across public health initiatives.

Qualifications

• Requires RN license in California and Public Health Nursing Certificate.
• Experience in immunization programs, disease surveillance, and case investigations preferred.
• Ability to supervise staff and coordinate multidisciplinary teams.

Responsibilities

• Plan, supervise, and evaluate immunization and communicable disease activities.
• Coordinate Vaccines for Children (VFC) program requirements and vaccine management.
• Conduct case and contact investigations and provide provider education.
• Prepare reports and monitor grant scope of work deliverables.
• Support outbreak investigations and community collaborations.
