Job summary

Cass County Public Health is seeking a Public Health Nurse to promote and protect the health of Cass County residents using nursing, social and public health sciences through education, prevention, and advocacy.

The role conducts health assessments in homes, clinics, and community settings, links people to care, and may provide direct nursing services; weekend home visits may be required. Emphasis on health education and environmental protection.

Qualifications

• Current Certification as a public health nurse in Minnesota and RN licensure in Minnesota; PHN cert to be obtained within six months of employment.
• Valid driver’s license required.

Responsibilities

• Perform initial and ongoing health assessments in homes, clinics, and communities.
• Link clients to providers and ensure access to care; provide nursing care per physician plan.
• May provide direct skilled nursing and weekend home care coverage as needed.
• Promote healthy communities through education and outreach; mitigate environmental health risks.
• Summarize data and collaborate with partners to address risk and develop plans.
