Job summary
Georgia Department of Public Health is seeking a Public Health Registered Nurse, Level 1, 2, or 3, to provide home visiting and care coordination for perinatal patients and infants across the 14-county district.
This 1 full-time position may be housed in Baker, Dougherty, or Early County, with a salary in the K/L/M bands and comprehensive benefits including leave, retirement, and health plans.
Qualifications
- Associate degree in nursing from an accredited college or university.
- Completion of PHN course within one year of hire.
- Current Georgia RN license and CPR certification.
Responsibilities
- Provides home visiting services within the 14-county health district to identify, assess, and provide care coordination for eligible perinatal patients and qualifying infants.
- Identifies high-risk populations and tailors interventions based on needs and available resources.
- Maintains HIPAA privacy and security standards and delivers outstanding customer service.
- Under supervision, provides professional nursing care per protocols and promotes the program through community engagement and outreach events.
- Provides referrals and linkages to appropriate services when medical or environmental risks are identified.
- Must maintain CPR certification and respond to public health emergencies.
