• Maintains open communication with case manager, therapist, and other healthcare professionals to facilitate continuity of care and discharge planning.
• Maintains productivity standards and adjusts work hours to meet patient needs.
• Participates in department meetings and performance improvement initiatives to maintain competencies.
• Monitors patient response to treatment and collaborates with the Physical Therapist for adjustments.
• Educates patients and caregivers on identified needs and implements treatment plans.
• Documents interventions and patient progress accurately and timely.
• Other duties as assigned.
