EmpowerMe Wellness in Jerseyville, IL is seeking licensed therapy professionals to deliver therapy and wellness services for older adults within our EmpowerMe 360 program, helping residents live stronger, safer, and more independent lives.

You will conduct assessments, design personalized plans, and provide evidence-based interventions as part of a collaborative team, coordinating with caregivers and telehealth when needed. This role emphasizes impact and compassion.

• Current professional license in your therapy discipline (PT, PTA, OT, COTA, or SLP)

Responsibilities

• Maintain professional and ethical standards in line with company initiatives
• Contribute to program development and continuous quality improvement
