Job summary

Relient Health is seeking a licensed Physical Therapist Assistant (PTA) for a Skilled Nursing Facility in Portland, OR. This full-time role offers a supportive team, great benefits, and opportunities to help patients regain independence.

Responsibilities include providing therapy under the supervision of a licensed PT, developing treatment plans, documenting progress in EMR, and collaborating with PTs, OTs, SLPs, and nursing staff to promote patient mobility.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current OR license (or eligibility to obtain).
• New grads and experienced PTAs welcome!
• Strong communication, documentation, and interpersonal skills.

Responsibilities

• Provide therapy services under the direction of a licensed Physical Therapist.
• Assist in developing and implementing individualized treatment plans.
• Document patient progress using EMR systems.
• Foster strong communication with PTs, OTs, SLPs, and nursing staff.
• Promote patient independence and mobility.
