Flagler-Health-and-Rehabilitation-Cente is seeking a Physical Therapist Assistant (PTA) for a PRN role in a Skilled Nursing Facility. You will support residents through rehabilitation, assist with mobility training, and help implement therapy plans.

Collaboration with therapists and nursing staff is essential. Ideal candidates have PTA certification or license for Florida, strong communication with elderly residents, and a compassionate, patient-centered approach to care.

Qualifications

• Completion of an accredited Physical Therapist Assistant program.
• Current PTA certification or license in the state of Florida.
• Strong communication and interpersonal skills, especially with elderly or rehabilitating residents.
• Ability to work collaboratively with Physical Therapists, nursing staff, and other care team members.
• Compassionate, patient-centered approach to care.
• Basic understanding of rehabilitation principles and SNF regulatory guidelines (preferred).
• Ability to lift, support, and assist residents safely during mobility and therapeutic activities.

Responsibilities

• Deliver high-quality patient care under the supervision of a licensed Physical Therapist.
• Assist residents with mobility training, gait training, transfers, and strengthening exercises.
• Implement individualized therapy treatment plans according to resident needs.
• Monitor resident progress and document outcomes accurately and timely.
• Provide education and support to residents and their families regarding therapeutic activities and home exercise programs.
• Maintain a safe and clean therapy environment, ensuring equipment is used appropriately.
• Collaborate with nursing staff, therapy teams, and interdisciplinary teams to support overall care plans.
• Encourage resident participation and promote independence in daily functional activities.
