Job summary

Renewal Rehab is seeking a licensed Physical Therapist Assistant for a full-time role in a SNF/Long-Term Care setting. You will work under the supervision of a licensed Physical Therapist to provide therapy services and coordinate patient referrals.

Qualifications

• Must be a licensed Physical Therapist Assistant to apply.

Responsibilities

• Provide physical therapy treatments under supervision of a licensed PT.
• Assist in identifying eligible patients for therapy and coordinate with PT for evaluation.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure all physician orders are obtained and up to date.
