Job summary

Crossroads Treatment Centers in Kentucky is seeking a Substance Abuse Counselor to support patients with opioid use disorder. You will provide individual counseling, conduct clinical assessments, and design and update treatment plans, including care coordination, referrals, and discharge planning.

The role requires managing a caseload and collaborating with nurses and providers to deliver holistic care. Expect a 29-hour week with a Saturday rotation and opportunities for advancement and training.

Qualifications

• General experience working with underserved populations in a clinical setting.
• Lived addiction experience is a plus.
• Certifications/licenses listed in Education are required or preferred.
• Ability to schedule and manage caseload within state regulations.

Responsibilities

• Provide individual counseling to highly complex patients.
• Conduct clinical assessments to determine treatment needs.
• Design and update the patient’s treatment plan, including care coordination, referrals, and discharge planning.
• Manage assigned caseload and ensure compliance with clinical treatment requirements.
• Collaborate with nurses and providers to deliver holistic care.
• Facilitate urine drug screening for patients.
• Participate in a Saturday rotation as part of a 29-hour weekly schedule.

It is expected for all clinical team members to participate in our Saturday rotation. The frequency of this rotation is determined by the current staff count.
