Job summary

Freedom at Home is seeking a Physical Therapist Home Health to evaluate patients in their homes and create individualized treatment plans. You will provide skilled PT services, educate patients and caregivers, and ensure timely documentation including OASIS.

Collaboration with physicians and nurses is essential to monitor progress and adjust plans. Ideal candidates have an accredited PT program, an active Michigan PT license, CPR certification, and at least 1 year of PT experience.

Qualifications

• Minimum of 1 year of PT experience.
• Current Michigan Physical Therapist license.
• Current CPR certification.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans.
• Provide skilled physical therapy services.
• Educate patients and caregivers on exercises, safety, and mobility.
• Complete OASIS and clinical documentation in a timely manner.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Monitor patient progress and adjust treatment plans as needed.
