Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a dedicated Home Health Physical Therapist for the O'Fallon, IL area. You will conduct 1:1 home-based evaluations and treatments, set goals, and coordinate with families and the care team to maximize independence.

Qualifications

• Graduate from a CAPTE-accredited Physical Therapist education program.
• Active Physical Therapist license in the state and CPR certification.
• Two years of adult PT experience; home health preferred.
• Excellent observation, verbal and written communication, problem solving, and math skills.

Responsibilities

• Perform PT evaluations and treatments per physician orders.
• Establish measurable goals and treatment plans to improve independence and function.
• Maintain communication with the treatment team, physician, patient, and family.
• Document services thoroughly for proper billing.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Graduate of a Physical Therapist Education program approved by CAPTE
• Current Physical Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
• Two (2) years of experience in adult physical therapy, home health experience preferred
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist
