Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Home Health Physical Therapist to join our Charlotte, NC team. You will provide expert, patient-centered care in home settings, evaluating and treating patients per physician orders.

This role offers a Monday–Friday schedule, 8am–5pm, with 1:1 patient care, team support, opportunities for advancement, and a supportive environment where empathy and clinical skills shine.

Qualifications

• Graduate of a Physical Therapist Education program approved by CAPTE.
• Current Physical Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire.
• Two (2) years of experience in adult physical therapy; home health experience preferred.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist.

Responsibilities

• Evaluate and treat patients per physician orders, including adaptive equipment.
• Set measurable goals and plans to maximize independence and function.
• Coordinate with treatment team, physician, patient, and families.
• Document services timely for accurate billing.
• Supervise PTAs and PT Aides per state practice act.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act
