Job summary

UnitedHealth Group is seeking a Physical Therapist to provide skilled PT services in patients' homes, promoting mobility, safety and independence. You will collaborate with patients and an interdisciplinary team to support recovery and prevent hospitalizations.

Responsibilities include home-based PT care, periodic reexaminations, discharge planning, and adapting interventions as needed. A strong license, CAPTE-approved degree, CPR, and 1+ year in home health are required.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from CAPTE-approved physical therapy program.
• 1+ year experience in home health care; if less than 1 year, mentoring in place.
• Current CPR certification.

As the Physical Therapist (PT) you will be responsible for providing skilled physical therapy services to patients in their homes to promote mobility, restore function, improve strength and balance, and enhance overall safety and independence. The Physical Therapist works collaboratively with patients, caregivers, and an interdisciplinary care team to support recovery, prevent hospitalizations, and improve quality of life within the home setting.

Primary Responsibilities:

• Responsible for helping patients maintain, restore and improve movement, activity and function.
• Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
• Other duties as assigned.

Required Qualifications:

• Current license to practice physical therapy specific to the state you’re assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• 1+ related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.

The salary for this role will range from $80,864 to $121,296 annually based on full-time employment. We comply with all minimum wage laws as applicable.
