Job summary

ARMStaffing is seeking a Physical Therapist to join a home health team serving the White River Junction, VT area and surrounding regions. The role involves evaluating patients, designing treatment plans, and delivering therapy in patients\' homes as part of a coordinated care effort with the Visiting Nurse and Hospice for Vermont and New Hampshire.

The ideal candidate holds a BS in Physical Therapy and VT/NH licensure (or compact license) with at least one year of home health experience.

Qualifications

• BS in Physical Therapy.
• Must hold a VT and NH license, or an unencumbered compact license.
• One year of PT experience in a home healthcare setting.

Responsibilities

• Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
• Carry out treatment programs according to agency policies and professional standards of care.
• Document evaluations, treatment goals, and objectives accurately.
• Communicate effectively with all members of the healthcare team.
• Teach patients and family members the rationale for treatments and procedures.

Job Description

As a Physical Therapist at Visiting Nurse and Hospice for Vermont and New Hampshire, you will have the opportunity to create meaningful relationships with your patients while being an integral part of their care team. Your role will involve conducting evaluations, developing treatment plans, and providing therapy in a home health setting.

Responsibilities

• Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
• Carry out treatment programs according to agency policies and professional standards of care.

Required Experience / Certifications / Licensure
