Job summary

Harmony Health and Rehab is seeking a dedicated Physical Therapist Assistant (PTA) to deliver high-quality therapy in a 120-bed skilled nursing environment. You will work under a licensed Physical Therapist to improve mobility, reduce pain, and enhance independence for residents.

Qualifications include graduation from an accredited PTA program, current licensure, CPR certification, and prior SNF/long-term care experience is preferred.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current PTA license/certification.
• CPR certification required or obtained upon hire.
• Experience in skilled nursing or long-term care setting preferred.
• Strong clinical, communication, and teamwork skills.

Responsibilities

• Carry out treatment plans designed by the Physical Therapist, including exercises, gait training, balance activities, and functional mobility tasks.
• Assist residents in improving strength, ROM, and endurance.
• Monitor and document residents’ treatments, progress, and responses per standards.
• Communicate with supervising PT about status, changes, and care plan adjustments.
• Provide education to residents, families, and caregivers on exercise programs and safety.
• Adhere to facility policies and regulatory requirements, including infection control.

The Physical Therapist Assistant (PTA) will work under the supervision of a licensed Physical Therapist (PT) to deliver high-quality therapy services that help residents improve mobility, restore physical function, reduce pain, and enhance independence in daily activities.

Physical Therapist Assistant Key Responsibilities:

• Carry out treatment plans designed by the Physical Therapist, including therapeutic exercises, gait training, balance activities, and functional mobility tasks.

• Assist residents in improving strength, range of motion, and overall physical endurance.

• Monitor and document residents’ treatments, progress, and responses in accordance with facility and regulatory standards.

• Communicate effectively with the supervising PT regarding residents’ status, changes, and potential adjustments to care plans.

• Provide education to residents, families, and caregivers on exercise programs, mobility techniques, and safety measures.

• Adhere to facility policies, professional practice standards, and regulatory requirements, including documentation and infection control.

Physical Therapist Assistant Qualifications:

• Graduate of an accredited Physical Therapist Assistant program.

• Current and valid licensure/certification as a PTA.

• CPR certification required (or must obtain upon hire).

• Experience in a skilled nursing or long-term care setting preferred.

• Strong clinical, communication, and teamwork skills.
