Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) to manage a field-based caseload and deliver high-quality in-home skilled care. You will coordinate with the clinical team, drive patient progression, and implement a robust plan of care focused on gait, transfers, strengthening, and home safety.

You will operate within a productivity-based model (75.00 points) and earn $100,000–$130,000 annually, plus a $2,000 sign-on bonus for eligible hires through Sept 30.

Qualifications

• Active Texas PT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Comfortable escalating changes to RN/MD and documenting rationale.
• Strong communication and thorough documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Valid driver’s license and comfort with daily travel across territory.
• Experience supervising PTAs and collaborating on care plans.

Responsibilities

• Own the caseload as a case manager with therapy plan, progression and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a plan of care: gait/balance, transfers, strengthening, functional mobility.
• Set measurable goals, track progress, adjust treatment as needed.
• Drive multidisciplinary coordination and keep the care team aligned.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in HCHB.

What you will do:

• Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes - not just individual visits.




• Complete comprehensive in-home assessments and deliver skilled PT visits.

• Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.




• Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.




• Manage treatment and outcomes directly, including PTA supervision where appropriate.




• Coach patients and caregivers with practical home programs that actually get done.




• Document promptly and thoroughly in Homecare Homebase (HCHB) - defensible, efficient, and consistent.

Requirements

Must-haves

• Active Texas PT license (in good standing)




• Strong clinical judgment and comfort working independently in the field




• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly




• Strong communication and documentation




• Reliable follow-through on orders, frequencies, and patient goals




• Current driver's license and comfort with daily travel across assigned territory




• Able to carry/transport basic therapy equipment as needed
