Qualifications

• Master’s degree in psychology, social work, marriage and family therapy, or related field.
• Minnesota licensure required: LP, LICSW, LMFT or LPCC.
• Minimum of one year of experience in psychotherapy or related field.
• Compliance with Minnesota sexual exploitation laws for staff.

Responsibilities

• Assess and evaluate patients through psychological assessments and social histories.
• Establish collaborative treatment plans with goals and implement care with the physician and team.
• Provide case management including discharge and safety planning.
• Coordinate nursing home referrals and county screening processes.
• Complete documentation of assessments and treatment progress.
• Work in outpatient setting with some remote work flexibility.
