Qualifications

Responsibilities

• Assist all patients with ADLs and daily living activities.
• Perform 15-minute safety checks on all patients.
• Assist with admissions and discharges while maintaining patient safety.
• Assist patients at meals with their food trays and ensure safety in the Dining Room.
• Perform accu-checks and vital signs as needed.
• Carry out patient care tasks as directed by the Charge Nurse.
• Intervenes verbally and physically with patients who are assaultive or self-harming.
• Document in flow sheets: 15-minute checks, graphs, restraint flow sheets, I&D flow sheets, patient education and diabetic flow sheets.
• Assist recreational staff with activities as needed.
• Attend 75% of staff meetings and 100% of mandatory trainings.
