Job description

SUMMARY OF POSITION

Responsible for providing primary nursing support to medication services/clinic clients.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Maintains standards of agency and credentialing policies and procedures.
  • Participates in on-going QI activities.
  • Participates as member of UM Per Panel.
  • As member of Medical Service Team will actively participate in Medical Service meetings and program related meetings.
  • Completes medical records and other documentation in a timely and accurate manner.
  • Coordinates with psychiatrists and pharmacies for medication management.
  • Consistently meets billable expectations.
  • Coordinates and facilitates telehealth appointments between clients and providers.
  • Completes prior authorizations for medications.
  • Maintains a safe, hygienic service provision environment.
  • Administers injectable medication and provides appropriate client education with respect to medication.
  • Takes calls from hospitals, doctors, nurses and other mental health professionals to coordinate care of client.
  • Triage walk‑in clients and provide nursing assessment, supportive therapy, crisis intervention and exploration of therapeutic options.
  • Facilitates and participates in offsite medication clinics in service area.
  • Refers clients to therapists, Day Programs, Partial Hospital, Hospital or other providers.
  • Works in a cooperative and problem‑solving manner with other agency departments and community agencies.
  • Represents agency in a cooperative, positive manner, identifying and reporting any concerns regarding improvement of the service delivery system.
  • Provides back‑up for crisis to non‑medical QMHP.
  • Maintains knowledge of agency technology programs and/or systems needed to do the job and uses them effectively.
  • Assists all Med Team members as needed.
  • Other duties and/or responsibilities may be assigned by supervisor.
EDUCATION & CERTIFICATIONS
  • Registered Nurse preferred, LPN required.
  • CPR/First Aid certification required.
  • Active Driver’s License with minimum state required insurance.
  • Experience Required: 2+ years of similar position‑related experience preferred.
REQUIRED KNOWLEDGE
  • Knowledge gained through prior employment and/or education related to targeted health, along with chronic disease processes, especially related to obesity, diabetes, hypertension, COPD/asthma, and heart disease.
  • Experience in behavioral health and/or substance use fields preferred.
SKILLS & ABILITIES
  • Excellent phone and verbal communication skills.
  • Proficient computer skills to include Microsoft Word and Outlook.
  • Experience with electronic medical records.
  • Proficiency with point‑of‑care metabolic screening testing and vital sign collection used for metabolic syndrome screenings.
  • Excellent patient/consumer education skills.
  • Excellent organization and prioritization skills.
