Job summary

Holcomb Brand is seeking a Psychiatric Rehabilitation Worker in Wyomissing, PA, to work directly with individuals facing mental health and behavioral health challenges. The role focuses on developing skills, confidence, and supports for productive, independent living.

Location is Wyomissing, PA with a full‑time schedule (Mon–Fri, 8:30 am–4:30 pm; flexible scheduling). The Mobile Psychiatric Rehabilitation program offers ongoing training and opportunities for professional growth.

Qualifications

• Bachelor’s or associate’s degree in a human services field with at least 1 year of experience.
• Equivalent combinations of education and experience may be considered.
• PA Criminal Clearance, PA Child Abuse Clearance (if applicable), Valid Driver’s License, not listed on Medicaid/Medicare Exclusion lists.

Responsibilities

• Provide recovery‑oriented psychiatric rehabilitation services based on individualized service plans.
• Support consumers with daily living skills, medication education, budgeting, self‑care, and supportive counseling.
• Maintain a small caseload (4–5 consumers) while promoting independence and personal growth.
• Accompany consumers to medical appointments and complete daily van runs.
• Meet regularly with individuals in their homes, schools, and community settings.
• Build trusting relationships that encourage personal growth and success.
• Assist clients in overcoming barriers that may interfere with their ability to live productive and fulfilling lives.
• Administer medications in accordance with DPW standards.
• Respond appropriately to crises and support healthy relationships.
• Complete required documentation and maintain compliance with 2600 regulations.
• Collaborate with program staff while working independently as needed.
• Assist with light housekeeping, meals, and other hands‑on support as required.
