Job summary

EvergreenHealth is seeking an Occupational Therapist to deliver skilled home health care across assigned WA cities, including Renton. You will work with an interdisciplinary team to assess, plan, implement, and evaluate patient care in the home setting.

The role requires WA OT licensure, an active driver’s license, and a current BLS certification. Willingness to travel among assigned communities and collaborate with preceptors in a supportive environment is essential.

Qualifications

  • Current OT licensure to practice Occupational Therapy in Washington State.
  • Obtain an NPI number within 30 days of starting.
  • Recent clinical experience in medical/surgical unit, rehab, or home health.
  • WA State Driver’s license and proof of insurability.
  • Current Healthcare Provider BLS certification by date of hire.
  • Reliable transportation.

Responsibilities

  • Provide, coordinate and oversee skilled intermittent care to patients in the patient’s home.
  • Regularly evaluate and re-evaluate patient status.
  • Initiate, develop, implement and revise the patient’s plan of care (POC).
  • Address all problems identified in the POC.
  • Educate patient/family/caregiver about the condition and needs.
  • Communicate with other disciplines involved in the case as warranted.
  • Report signs/symptoms of change in patient condition to physician and case manager as needed.
  • Reassess the patient within 48 hours after change in condition or discharge from hospital stay.

Wage Range: $46.33 - $74.14 per hour

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity

This position will drive to patients living in the following assigned WA state cities: Kirkland, Duvall, Issaquah, Renton, Bellevue.

Job Summary

Responsible for assessing, planning, implementing, evaluating and providing skilled care to patients requiring occupational therapy services to patients in the home, in conjunction with the patient, family and the interdisciplinary team, as ordered by the physician. The OT will be responsible for clinical outcomes as well as industry benchmarks.

Primary Duties:
  1. Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient’s home.
  2. Regularly evaluates and re-evaluates patient status.
  3. Initiates, develops, implements and revises the patient’s plan of care (POC).
  4. Addresses all problems identified in the POC.
  5. Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient’s condition.
  6. Communicates with other disciplines involved in the case as warranted.
  7. Reports signs/symptoms signaling a change in patient condition to the patient’s physician and case manager as needed.
  8. Reassesses the patient within 48 hours following a change in patient condition or discharge from a 24-hour or longer hospital stay, or as ordered by the physician.
License, Certification, Education or Experience:

REQUIRED for the position:

  • Current OT licensure to practice Occupational Therapy in Washington State
