Resilient Health in Phoenix, AZ is hiring a Psychiatric Nurse Practitioner to diagnose, assess, and prescribe meds across the lifespan, including children and adolescents, with telemedicine and in-person visits. You will develop integrated treatment plans and deliver patient-centered care with follow-up.

• APN certification required
• Master’s in PMHNP
• Experience in behavioral health preferred
• Valid driver’s license
• CPR/First Aid certifications or ability to obtain within 30 days
• Fingerprint Clearance Card or ability to obtain within 90 days
• Three-year Motor Vehicle Record that meets insurer requirements

Responsabilidades

• Plan, organize, and provide ongoing medication management for new and existing patients
• Conduct initial psychiatric evaluations, periodic medication management and monitoring visits
• Document patient visits in accordance with agency standards
• Review diagnostic test results and document the course of action taken
• Renew and refill medication to avoid lapses in treatment
• Provide emergency face-to-face counseling and/or crisis intervention as needed
• Develop and maintain effective working relationships with community agencies, case managers, PCPs, psychiatrists, and other clinical team members
• Participate in clinical staffing and individual service planning with case managers and stakeholders
• Educate patients on disease process and preventative practices
• Order diagnostic tests and screening evaluations
• Offer case consultation and clinical training to team members
• Assist clients in pursuing effective treatment goals
• Identify types and amounts of support clients require to remain independent
• Maintain information on community resources with team support
• Provide education and support to family members to enhance awareness of illness

About Our Nurse PractitionerPosition:

The Psychiatric Nurse Practitioner position entailsdiagnosis, conducting assessments, and prescribing medications for patientsacross the lifespan, including children and adolescents, havingpsychiatric-mental health conditions, including those with serious mental illness(SMI). The role also includes developing an appropriate, integrated healthcaretreatment plan and providing effective patient care management with properevaluation and follow-up care. This position will provide telemedicineand in-person services based on patient and clinic needs.

• Assist clients in identifying and pursuingeffective treatment goals
• Identify the types and amounts of supportclients require to remain independent despiteacute and chronicdiseases
• Maintain information on communityresources for clients' assistance and support, with support from teammembers
• Provide education, assistance, and supportto family members and others involved with the client to enhance awarenessof mental and emotional illness and to increase their ability tocontribute to the rehabilitation process.

Requirements for ThisPosition:

• Advanced Practice Nurse PractitionerCertification from AZ State Board of Nursing as a Psychiatric/MentalHealth Nurse Practitioner (across the lifespan) required
• Master's in PMHNP
• Experience in a behavioral health settingpreferred
• Valid driver's license
• Valid CPR/First Aid certifications orability to obtain within 30 days of hire
• Valid Fingerprint Clearance Card orability to obtain within 90 days of hire
• Three-year Motor Vehicle Record that meetsour auto insurance carrier's coverage requirements

What This Position Pays: $150,000 - $160,000, depending on years of NP experience
