Job summary

MindCare Solutions is seeking a dedicated Psychiatric Nurse Practitioner to join our team on a part-time basis in Clearfield, PA. You will conduct comprehensive psychiatric evaluations, manage medications, and collaborate with interdisciplinary teams to deliver value-based care across geriatric and LTC settings.

The role requires an active PMHNP-BC, PA license, and DEA registration; travel up to 50% to partner facilities; up to 8 days per month. Compensation is dependent on experience.

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited program specializing in Psychiatric Mental Health
• Current and unrestricted PMHNP license in the state of PA
• Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
• Active and unrestricted DEA registration
• Clinical expertise in psychiatric evaluations and medication management
• Interpersonal and team collaboration skills for cross-functional care
• Community-focused with awareness of social determinants of health
• Tech-savvy with telehealth/ digital tools
• Willingness to travel within assigned regions to support partners

Psychiatric Nurse Practitioner

MindCare Solutions is seeking a dedicated and compassionate Psychiatric Nurse Practitioner (PMHNP) to join our dynamic team on a part-time basis. As a leading prover of mental health services, we are committed to providing exceptional care and support to our geriatric patients and residents who are facing mental health challenges in the LTC community.

Location: Clearfield, PA

Hours: Part-Time, up to 8 days per month

Compensation: Dependent on experience

As a PMHNP with MindCare Solutions, you will collaborate with interdisciplinary teams to deliver high-quality, value-based psychiatric care across a variety of clinical settings. You will focus on proactive, evidence-based treatment strategies that improve the health and well-being of individuals while strengthening the capacity of partner organizations. This dynamic role emphasizes adaptability, innovation, and a patient-centered approach to behavioral health care. Reporting to Chief Medical Officer.

Essential Duties and Responsibilities

• Conduct Comprehensive Psychiatric Evaluations: Perform thorough psychiatric assessments to establish a baseline, manage psychotropic medications, and deliver personalized, need-based care.
• Ongoing Medication Management: Develop and adjust treatment plans, including medication management, based on patient progress and input from the interdisciplinary team.
• Collaborative Care Planning: Partner with case managers and interdisciplinary teams to create and refine care plans, ensuring a seamless, integrated approach to behavioral health management.
• Team Engagement: Participate in regular team meetings to evaluate new referrals, review active cases, and adjust care plans based on patient progress and organizational goals.
• Engagement in Quality Programs: Contribute to quality initiatives such as performance improvement programs, medication reviews, and outcome optimization strategies.
• Travel: Travel up to 50% of the time to meet with healthcare partners, attend industry events, and provide consistent support across clinical settings.

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited program specializing in Psychiatric Mental Health
• Current and unrestricted PMHNP license in the state of PA
• Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
• Active and unrestricted DEA (Drug Enforcement Administration) (We will help obtain)
• Clinical Expertise: Proficiency in psychiatric evaluations, psychotropic medication management, and evidence-based treatment planning.
• Interpersonal and Team Collaboration Skills: Strong ability to build relationships and work collaboratively with interdisciplinary teams in diverse care settings.
• Community-Focused: Awareness of social determinants of health and access to resources to address holistic patient needs.
• Tech-Savvy: Comfortable using electronic health records (EHRs), telehealth platforms, and other digital tools to enhance care delivery.
• Flexibility and Mobility: Willingness to travel within assigned regions to provide direct care and support to partner organizations, ensuring accessibility and continuity of services.
