FamilyWell Health & FamilyWell Academy seeks an independently licensed psychiatric nurse practitioner to provide telehealth psychiatric evaluations and medication management for perinatal and peri/menopause patients across the U.S. This role emphasizes collaborative care within OB/Gyn and family medicine settings.

You will conduct pre-conception consultations, manage caseloads using the care registry, and support care teams with education on diagnosing and treating behavioral health conditions

Qualifikationen

• Must hold active CT PNP license and DEA registration.
• Ability to work independently without supervision.
• Experience or interest in perinatal or collaborative care mental health.

• Provide pre-conception medication consultation and psycho-education for individuals trying to conceive
• Perform asynchronous caseload consultations by reviewing clinical information and medical records from the care team
• Complete documentation for consultations and direct visits within 24 hours
• Oversee assigned patient caseload using the care registry and prioritize patients for review and treatment adjustment
• Provide educational support to care teams on diagnosing and managing behavioral health conditions
• Offer ad hoc telephone, email, or telehealth consultations to OB providers for complex patients during business hours
