Access Healthcare Associates, part of Brigade Health, seeks a Psychiatric Mental Health Nurse Practitioner to join a mobile geriatric psychiatry team providing care in homes and senior living communities. You will balance independent practice with strong team collaboration to redefine seniors’ mental health experiences.

Qualifications

• Active PMHNP license in the state you will practice.
• Experience in geriatric psychiatry, long-term care, or mobile healthcare preferred.
• Strong clinical judgment and ability to work independently with remote support.

Responsibilities

• Conduct comprehensive psychiatric evaluations for geriatric patients in home and facility-based settings.
• Diagnose and manage mood disorders, anxiety disorders, dementia-related behaviors, and other psychiatric conditions.
• Prescribe and manage psychotropic medications with consideration of geriatric physiology.
• Provide crisis intervention, behavioral guidance, and family education when indicated.
• Collaborate with primary care providers, therapists, and facility care teams for coordinated care.
• Build trusted relationships with patients, caregivers, families, and community partners.
• Maintain consistent, accurate clinical documentation and follow-through.

Your Responsibilities Include

• Conductingcomprehensive psychiatric evaluations for geriatric patients in home and facility-based settings.

• Prescribing and managing psychotropic medications with careful consideration of comorbidities and geriatric physiology.
• Providing crisis intervention, behavioral guidance, and family education when clinicallyindicated.
• Collaborating with primary care providers, therapists, and facility care teams to ensure coordinated, whole-person care.
• Building trusted relationships with patients, caregivers, families, and community partners.
• Maintainingconsistent,accurate clinical documentation and follow-through.

Competitive Compensation

• Guaranteed Base Salary + Productivity
• Total annual compensation range: $150,000–$200,000+ , based on experience, caseload, and availability.
