Job summary

Port Gamble S'Klallam Tribe seeks a qualified Psychiatric Mental Health Nurse Practitioner to join the Integrated Clinical Care Team. The PMHNP will diagnose, treat, and manage psychiatric health care, prescribing medications as appropriate and coordinating with staff and patients to improve outcomes.

The position requires a Master’s/Doctoral degree in Nursing, board certification, WA state ARNP/RN licensure, and at least 3 years of PMHNP experience, with occasional travel and situational remote

Job Details

Job Location: Port Gamble S'Klallam Tribe - Kingston, WA 98346.

Position Type: Full Time - 30 - 40 hrs/week.

Education Level: Certification - Required.

Salary Range: $120,784.00 - $187,215.00 Salary/year.

Travel Percentage: Occasional travel to trainings.

The PMHNP is a part of the Integrated Clinical Care Team. They will diagnose, conduct, and prescribe medications for patients having psychiatric-mental health problems and those likely to have them.

The psychiatric mental health nurse practitioner is responsible for providing psychiatric services and physical and psychosocial examination of their patients. Their role also includes developing appropriate treatment plans and providing effective management of patient care with proper evaluation. They may also function as educators and consultants to staff, families, and groups and participate in developing policies and healthcare reforms, improving service quality, and evaluating practice.

Duties Include

Clinical duties

• Provides crisis intervention as needed, including safety planning, relapse prevention planning, referral to community resources, and same-day evaluation and possible stabilization.
• Responsible for providing outpatient care to patients showing a change or decline in mental status, chronic or urgent medical conditions.
• Take part in an interdisciplinary team that provides primary care to employees and patients
• Conduct initial evaluations and collaboratively develop treatment plans for medication, therapy, or a combination of both with patients.
• Proactively follow up with patients at least monthly to assess the efficacy of recommended interventions and ongoing care until treatment goals are achieved.
• Utilize a combined medication and therapy approach.
• Must be comfortable providing various modalities of talk therapy and incorporating behavioral interventions.
• Collect and document patients' relevant health history, risk factor assessment, ordering of therapeutic and diagnostic tests; and attend consultations and appointments.
• Prescribe medications following the clinic’s policy, formulary, and relevant regulations.
• Consult with physicians on cases that are beyond the scope of qualification of PMHNP.
• Enter all relevant patient data in medical records and case presentations.
• Formulate, execute, and review healthcare education delivered to patients and families for disease prevention.
• Ensure smooth communication with other health professionals to effectively deliver patients high-quality, collaborative care.
• Consult with the clinic’s medical and behavioral health staff to help improve patient service delivery.
• Practices in an independent role while consulting and collaborating with department staff and community partners.
• Presents client cases in regularly scheduled case staffing meetings to discuss status, updates, collaboration, referral, deposition, consultation, and recommendations.
• Participates in professional development and maintains required licensure and certification.

Policy Oversight/Development

• Provide leadership and direction in developing or modifying policies and procedures. Communicate changes to staff and monitor compliance.
• Monitors and implements quality assurance reviews and improvements.
• Ensure that all assigned services are compliant with all PCHS policies, procedures and guidelines, as well as applicable federal, state and local laws and regulations.

Community Development

• Supports Tribal community development efforts by participating in special events and other efforts, applying professional knowledge and skills.

Personal Contacts

• Maintains regular contacts of a positive nature with patients, patients' families, staff, community members, Tribal staff, various outside agencies, and others to accomplish program objectives and preserve good public relations.
• Reporting is mandatory for tribal employees and contractors with the tribe who perform services to the community in the areas of education, health and human services, law enforcement and members of the clergy.

Remote Work

This position is eligible for situational remote work as needed. The schedule must be discussed with and approved by supervisor and director and may be subject to change depending on role and department need.

Experience

• Experience working within American Indian and/or Alaska Native communities preferred.

Physical Requirements

• Regularly required to finger, handle, reach with hands and arms
• Frequently required to walk, sit, stand.
• Occasionally required to lift and move up to 25 pounds.
• Normal audio and visual acuity
• Moderate physical activity. Requires handling of average-weight objects up to 25 pounds or standing and/or walking for more than four (4) hours per day.
• Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment.
• Will work with blood or blood-borne pathogens and will require applicable training.

Travel Requirements

• Local, Statewide, and out-of-state travel may be required.
