You will collaborate with psychiatrists, therapists, primary care providers, and care managers to ensure coordinated, compassionate care. Must be licensed in NY or eligible and CPR certified; strong communication and teamwork are essential.

Qualifications

• LPN license in NYS required
• CPR certification required
• Strong assessment and communication skills
• Ability to work in a behavioral health setting

Responsibilities

• Obtain and document vital signs accurately to guide the treatment team.
• Administer long-acting injectable medications and collect bloodwork as directed.
• Educate patients and families about diagnoses, medications, and care plans.
• Coordinate care with psychiatrists, therapists, PCPs, and case managers.
• Handle patient calls and refill requests, including prior authorizations as needed.
• Maintain compliant, timely documentation in the EHR and ensure HIPAA compliance.
