Qualifications

• Assess patients' medical, social, emotional, financial, spiritual, and support systems.
• Complete assessments within 72 hours of admission.
• Ensure licensed professionals sign off on assessments for approval.
• Develop discharge and care management plans collaboratively.

Responsibilities

• Advocates for patients, ensuring understanding of medical care choices and documenting interventions.
• Collaborates with interdisciplinary teams for planning, service provision, and discharge planning.
• Monitors and evaluates social services on inpatient psychiatric units, providing counseling to patients.
