Job summary

Qualifications

• CPR/BLS certification required within 30 days of start date.
• First Aid certification required within 30 days of start date.

Responsibilities

• Assesses and treats children, adolescents, and adults seeking psychiatric and/or chemical dependency treatment.
• Communicates patient, resident, family needs to physicians accurately and completely.
• Familiarizes self with the plan of care and provides relevant insight into overall success or areas of improvement.
• Collaborates with referral sources, patients, families, and team members regarding care coordination.
• Provides follow-up actions as needed.
• Serves as the initial contact for patients, families, referral sources and/or allied agencies seeking intervention or support.
• Provides complete and accurate Intake Screening Reports of the patient and/or caller complaint and condition.
• Completes vital sign checks, mini-mental status exams, belonging checks.
• Leads patients in therapeutic and recreational activities.
• Monitors patients' physical and emotional wellbeing and reports unusual behavior or physical ailments to the medical staff.
• Observes patients for safety as directed by RN/LPN including rounding, direct observation, sitters.
• Assists patient with activities of daily living to include hygiene, laundry, dressing, personal care as needed.
