Job summary

Responsibilities

• Promotes the facility mission, vision and values.
• Conducts a full client inventory in the Intake area.
• Provide report to the nurse and tech taking the client in Intake.
• Do not leave the client unattended at any time.
• Identify contraband and dispose of it appropriately.
• Coordinate with nurse/family if home medications are involved.
• Orient patient and family to processes and unit routines.
• Assist on units between admissions with Intake Department.
• Orient patients to the unit and record vital signs.
• Record patient vital signs and intake/output; notify RN of changes.
• Provide general nursing care (transfers, ADLs).
• Observe patients for behavior, mental status and changes.
• Identify behaviors needing intervention and de-escalation.
• Escort patients for activities and tests.
• Perform patient safety checks and documentation.
• One-to-one observation for high-risk patients.
• Assist with restraints and seclusion when needed.
• Participate in patient care conferences.

Summary:

Responsibilities:

• Continuously observes patients' behavior, mental status and activities, notifies RN of any changes or unusual occurrences.
• Demonstrates the ability to identify behaviors that require intervention.
• Escorts patients when leaving the unit for activities and scheduled tests.
• Able to perform patient safety checks at scheduled times, documents appropriately.
• Responsible for one-to-one observation for high-risk patients.
• Able to assist with restraining a patient, placing a patient in seclusion.

• One - two (1-2) years experience as a nursing assistant; behavioral health setting preferred
