Job summary

Impact Health PC is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to deliver psychiatric care to residents in Skilled Nursing Facilities in the Christiana, PA area. You will assess, diagnose, and manage psychiatric conditions, developing individualized treatment plans and collaborating with physicians, facility staff, and care teams to ensure high-quality patient care.

• Active PMHNP license/certification in NJ, as applicable.
• Minimum 2 years of clinical experience providing mental health care to adults.

Responsibilities

• Develop evidence-based, individualized treatment plans.
