Job summary

• 3+ years of behavioral health, value-based contracting, or process and performance improvement experience required.

Responsibilities

• Collaborate with provider staff to develop and implement operational and clinical process improvements.
• Identify, develop, and establish key performance metrics to measure performance for value based contracts.
• Identify improvement opportunities based on KPIs and develop action plans.
• Develop and provide training and support to provider staff on performance metrics.
• Promote collaboration across provider groups to share best practices and improvements.
• Resolve system, process or resource issues addressing provider feedback and concerns.
• Perform other duties as assigned.

Job description

***NOTE: Travel (25%) is required throughout the Chicago metropolitan area, specifically targeting communities across Cook County and the surrounding collar counties (primarily DuPage, Lake, and Will counties)***

Position Purpose: Collaborate with healthcare providers to help them achieve high performance with their value based contracts.

• Collaborates with provider staff to develop and implement operational and clinical process improvements to help providers achieve higher performance
• Identify, develop, and establish key performance metrics to measure performance of providers related to their value based contracts.
• Identify improvement opportunities based on key performance indicators and develop action plans
• Develop and provide training and support to provider staffs to assist them in understanding performance metrics and implement operational and clinical improvements
• Promote collaboration across provider groups to enable sharing of best practices and performance improvement opportunities
• Resolve various system, process or resource issues to address provider feedback and concerns
• Performs other duties as assigned
• Complies with all policies and standards

Education/Experience:
Bachelor's Degree in Nursing, Medicine, or Health Sciences required.
Master's Degree in Nursing, Physician Assistant degree or graduate degrees in the fields of Pharmacy, Social Work, Counseling or Clinical Psychology preferred. 3+ years of behavioral health, value-based contracting, or process and performance improvement experience required.

Pay Range: $56,200.00 - $101,000.00 per year
