Job summary

Jawonio in New City, NY is seeking a Full Time Registered Nurse to provide nursing services for PROS program participants with mental health disabilities. You will collaborate with program providers to develop care plans and administer medications.

This is a fast-paced, supportive environment where you can make a real difference. The ideal candidate has a BSN, RN license in NY, BLS certification, and at least two years of related experience.

Qualifications

  • Daily access to an automobile and valid NY driver’s license.
  • Willingness to flex schedule (evenings/weekends) as needed.

Summary

Provide nursing services to PROS program participants with mental health disabilities by performing the following duties below. You will play a critical role in providing care to participants seeking to overcome barriers created by the mental health disability and restore functioning in various life roles. Essential Duties and Responsibilities
  • Collaborate with program providers to develop and update individualized recovery‑oriented nursing care plans for program participants, following health care standards to ensure the optimal care, safety and well‑being of the participant.
  • Provide support to program psychiatrist, including entering prescriptions into the e‑prescribing platform, maintaining appointment calendar and coordinating lab orders.
  • Coordinate with pharmacies to ensure all new prescriptions and refills are ordered and received in a timely manner. Complete prior authorizations as needed with insurance companies and follow‑up to obtain determination.
  • Provide health education to participants and families on diagnoses, symptoms, medications and side effects, including self‑care strategies to manage symptoms and side effects.
  • Facilitate program groups on health and wellness‑related topics. Provide individual supportive counseling as needed to program participants.
  • Prepare and administer long‑acting injectable medications to the participant, monitor for side effects and coordinate with pharmacies for refills or prior authorizations. Monitor expiration dates on all medications. Maintain necessary documentation related to the administration of medications, including keeping daily temperature log for refrigerated medications.
  • Maintain medication and medical supply inventories and inspection of medical equipment to ensure proper working order.
  • Ensure communication of all necessary information re: participants’ status, medication changes, etc. to program staff and residential program providers.
  • Conduct nursing assessments, triage and plan of care to identify emergent and ongoing health needs. Collaborate with primary care providers to support the health needs of participants. Request physical exams, lab reports, current prescriptions and health‑related documentation to ensure continuity of care.
  • Provide support to participants to promote health and wellness, including education about diabetes, metabolic syndromes and guidance on proper nutritional habits and smoking cessation.
  • Perform general nursing observations of participants regarding seizures, drug reactions, and other potential medical problems. Document these observations and notify appropriate professionals.
  • Refer participant to physician or other professionals as appropriate and inform them of specific needs re: prescriptions (i.e. time specific) and required documentation (consultation forms).
  • Provide crisis intervention to stabilize participants in acute distress using verbal de‑escalation and approved behavioral intervention techniques. Conduct risk assessment as needed for suicide or self‑harm.
  • Provide emergency care such as First Aid and/or CPR.
  • In the absence of supervisory personnel, assume a leadership role, directing staff and ensuring the safety and well‑being of participants.
  • Maintain accurate electronic health records, including completion of progress notes, assessments and incident reports. Document observations, medical appointments and other services received, plus all other pertinent medical information related to the health of the participant.
  • Prepare various reports as assigned, scheduled or necessary.
  • Conduct health‑related trainings to agency personnel.
  • Work collaboratively with other agency staff to provide seamless continuity of care between divisions.
Qualifications

Education/Experience: BSN and RN with two years related experience and/or training; or equivalent combination of education and experience.

Certificates and Licenses: NYS License as a Registered Nurse and BLS certification.

Additional Requirements:

  • Daily access to an automobile is required. A valid NY State Driver’s License and ability to drive and driving record acceptable for agency insurance coverage are required.
  • Remain current in all OMH and agency required trainings.
  • Candidate must be cleared and maintain acceptable record under the NYS Justice Center and NYS OMH mandated criminal background check process.
  • Must be clear and maintain an acceptable record under the Medicaid Fraud & Abuse clearance process.
  • Regular work schedule: Monday to Friday – 8:30am – 4:00pm. Requires flexing regular schedule to accommodate meeting and event times including occasional evening or weekend work.
