Job summary

HELP Foundation, Inc. is seeking a Program Specialist to ensure residential program services meet high standards.

Under supervision, you’ll write skill development and behavior support documents, lead team coordination with SSA’s, and analyze client data to guide interventions. You will assess informed consent, maintain consumer files, and participate in staff training, meetings, and incident investigations.

Qualifications

  • Must have a high school diploma or GED; a 4-year degree in a relevant field is preferred.
  • Minimum of 1-year experience in the field of Developmental Disabilities
  • Minimum of 1-year understanding, interpreting and implementing consumer ISP goals and objectives
  • Must have at least 2 year of experience using and/or reviewing service documents and records
  • Above average level of knowledge and understanding of provider licensure rules
  • Above average computer skills and ability to use Microsoft Office
  • Requires good time management and organization skills
  • Must possess exceptional verbal and written communication skills
  • Must be able to carry an Agency phone and be on-call
  • Must maintain current Med Pass and CPR/First Aid Certification; training available for CPR and Med Pass onsite
  • Must have a valid Ohio Driver’s License with less than 6 points
  • Successfully pass a background check that includes a BCI and FBI criminal history check

Responsibilities

  • Prepares necessary consumer programmatic documentation, including the writing of skill development documents, HPC’s, behavior support documents as needed.
  • Lead contact for the team with SSA’s
  • Conducts appropriate client assessments and collect and analyze data regarding the consumer’s response to interventions.
  • Assures that the consumer’s ability to provide informed consent is assessed and documented.
  • Responsible for assuring that client needs are met.
  • Responsible for attending client team meetings and is involved with the negotiations regarding client staffing needs.
  • Trains staff in the delivery of consumer service plans and communicate changes in plans to appropriate personnel at the agency.
  • Responsible for organizing and maintaining consumer files.
  • Participates in regular meetings reviewing client needs/quality of life and all client documentation.
  • Responsible for involving appropriate team members when consumer needs or circumstances change.
  • Ensures that client needs are accurately assessed in the ISP.
  • Participates in the investigation of incidents that affect consumers and take the lead role in the development of a prevention plan.
  • Demonstrates awareness of, follows all safety policies and procedures, and adheres to all recommended safe work practices.
  • Drives his/her personal auto between assignments, and transport clients as required.
  • Performs other duties or special projects as required or as assigned.

  • Opportunity to earn anywhere from $47,000 to $56,000 annually

Position Summary

  • Performs other duties or special projects as required or as assigned.
