Job summary
Easterseals NH is seeking a Program Specialist to join our Community Based Services team in Concord. This role supports individuals in residential homes and works closely with the Program Manager to review and monitor program goals.
You will serve as a Peer Guide for new Direct Support Associates, providing on-the-job training and ensuring documentation is complete. The job requires personal care and transportation duties as needed.
Qualifications
- Associates Degree in Human Services or related field with 1 year of Direct Support experience, OR a HS diploma with 2 years Direct Support experience.
- Valid driver’s license, a reliable vehicle and automobile insurance for transportation requirements.
Responsibilities
- Assist in developing, implementing and monitoring program goals with the Program Manager.
- Act as a Peer Guide for new Direct Support Associates, providing on-the-job training and orientation for at least six months.
- Provide direct support including personal care and transportation and help document required information.
