Job summary

JEVS is seeking a Program Specialist to coordinate care for individuals within the CLHS services, ensuring that the core principles are integrated into supports and services chosen for each person. Responsibilities include coordinating medical, behavioral, and social supports; training staff on ISP plans; participating in behavioral therapy, and ensuring 90-day medication reviews where appropriate.

Qualifications

• Participate in behavioral therapy processes and ensure 90-day medication reviews where appropriate; implement behavior plans and collect data.
• Problem-solve with the Program Director on programming issues.
• Assess functioning against outcomes and adapt program plans as needed.
• Maintain regular contact with supports Coordinators, family, medical staff, and day programs.
• Lead the team with assigned supports coordinators for each caseload.

The Program Specialist is the key position in the coordination of care for the individuals within the CLHS services. The Program Specialist is responsible for ensuring that the JEVS Core principles are integrated in the supports/services chosen by the individuals.

Responsibilities:

• Coordination in securing supports and services for the individual including: medical, therapists, financial resources, behavioral, daytime engagement activities, employment, communication, skill acquisition, socialization, recreation, etc.
• Coordinate specialized training of staff, specific to the individual's ISP program plans.
• Participate in behavioral therapy process and assure completion of ninety day medication review as appropriate. Assure implementation of behavior plans and track data collection.
• Participate in problem solving of issues and maintain open communication with the Program Director regarding all programming issues.
• Assess the individual's current level of functioning in relationship to an individual's outcomes and the individual's social and environmental supports and adapt program plan as needed.
• Maintain regular contact with the people involved in supporting the individual. This may include the supports coordinator, friends, family, neighbors, medical personnel, day programs and consultants.
• Acts as team lead in conjunction with assigned supports coordinators for each individual on caseload.
