Job summary

Bridgewell in Lynn, MA seeks a Program Manager of Individual Supports to oversee daily operations of in-home/community supports programs. You will ensure quality care, compliance with DDS and CARF standards, and effective staff management.

The role requires a Bachelor's degree and at least five years with developmental/psychiatric disabilities, including three in supervision. Strong leadership and crisis-responding capabilities are essential.

Responsibilities

  • Oversee daily program operations to ensure standards and alignment with agency mission.
  • Ensure quality of care and documentation including ISP, DDS regulations, and Bridgewell policies.
  • Manage staffing needs, scheduling, and coordination of per diem/agency relief staff.
  • Assist participants with medical/clinical appointments including transportation.
  • Provide leadership to staff to foster a positive work climate and cohesive team.
  • Promote professional growth through coaching, mentoring, and training.
  • Monitor quality of services and DDS/CARF compliance and program budgets.
  • Build and maintain relationships with funding sources, community groups, families, and other providers.
  • Provide coverage to individuals as needed and participate in after-hours on-call rotation.
  • Maintain core training certifications and knowledge in Addiction, Trauma-Informed Care, and Medication Management.
  • Other duties as assigned.

Summary: The Program Manager of Individual Supports oversees the daily operation of an In-Home Supports or Community Supports-based program. The oversight includes all programmatic administrative responsibilities required to run the program.

Schedule: Monday-Friday 9:00am-5:00pm (40 hours per week)

Essential Duties & Responsibilities:
  1. The Program Manager is responsible for the daily operations of the program, to ensure that performance standards are upheld and that all individuals and staff are working following agency requirements, mission, goals, and principles.
  2. Ensure program, agency, and accreditation standards are met concerning quality of care including individual behavior and treatment plans, ISP documentation, HCSIS, and other documentation as required, and in line with Bridgewell Policies & Procedures.
  3. Ensure the staffing needs of the program are met based on defined ratios and attendance of persons served. This includes coordination of work schedules, hiring of new staff, and coordinating the use of per diem and agency relief staff, as needed.
  4. Assist program participants with all medical and clinical appointments including transportation to and from appointments.
  5. Provide leadership to all staff by creating a positive work climate and creating a cohesive team environment.
  6. Promote the professional growth of all staff through coaching, mentoring, and continuous training.
  7. Ensure ongoing system for measuring quality of services and compliance with DDS regulations and Bridgewell policies and procedures.
  8. Ensure that services are maintained according to DDS, CARF, and all applicable regulatory standards. Ensure compliance with program budgets through consistent monitoring of financial analysis and various reports.
  9. Build and maintain positive relationships with funding sources, community groups, families, and other service providers.
  10. Provide coverage to individuals as needed
  11. Participate/manage after-hour's on-call rotation.
  12. Maintain core training certifications.
  13. Knowledgeable in the areas of; Addiction, Trauma Informed Care, and Medication Management
  14. Other duties as assigned
Required Education/Experience:
  1. Bachelor's degree in a related field
  2. Minimum of five years experience working with people with Developmental Disabilities and psychiatric disabilities, three years supervisory/administrative capacity.

  9. Have the ability to hear and differentiate tone, modulations, and escalation of voice, to identify and monitor escalated behaviors, and to maintain a safe environment.
  10. Ability to communicate with emergency services (i.e. Ambulance, fire, and police).
Physical Demands:
  1. While performing the duties of this job, the employee is frequently required to climb stairs.
  2. The employee must be able to lift and/or move up to 50 pounds.
  3. The employee must have a normal range of eyesight with or without corrective lenses, and have a normal range of hearing with or without corrective equipment.
