Job summary

BAYADA Fairfax Senior Living team seeks a relationship-focused Program Manager I to support our Home Health practice across senior living communities in Alexandria, Springfield, and Mount Vernon. This field-based role requires a clinically licensed professional who is organized, personable, growth-minded, and motivated by building partnerships and improving resident care.

You will coordinate with BAYADA Fairfax Senior Living office, community teams, and clinical staff to grow and maintain

Qualifications

• Active clinical license in good standing.
• Bachelor's degree required.
• Strong organizational, communication, and relationship-building skills.
• Reliable, self-directed approach managing work across multiple locations.
• Valid driver’s license and ability to travel throughout the territory.

Responsibilities

• Build and maintain strong relationships with senior living community partners, office teams, and clinical teams.
• Execute provider-of-choice commitments across the assigned territory.
• Serve as a consistent, responsive partner to community leadership and internal stakeholders.
• Identify opportunities to grow Home Health and outpatient business within the territory.
• Visit senior living communities regularly—up to five days per week—to understand needs and support service excellence.
• Coordinate communication and follow-through across teams.
• Represent BAYADA professionally with a culture of compassion, excellence, and reliability.
• Organize competing priorities and ensure commitments are completed on time.
• Participate in an initial 90-day orientation with clinical and senior living training.

Compensation: up to $90,000–$95,000 annually, plus opportunity for quarterly incentives

Our BAYADA Fairfax Senior Living team is seeking a relationship-focused Program Manager I to support our Home Health practice across senior living communities in Alexandria, Springfield, and Mount Vernon. This field-based role is ideal for a clinically licensed professional who is organized, personable, growth-minded, and motivated by the opportunity to strengthen partnerships and improve the experience of the individuals and communities we serve.

As Program Manager I, you will execute our provider-of-choice commitments and serve as a key connection point between the BAYADA Fairfax Senior Living office team, community teams, and clinical teams. You will help grow and maintain the business by building trusted relationships, coordinating priorities, and ensuring our services consistently meet the needs of our senior living partners and their residents.

What you’ll do

• Execute BAYADA Fairfax Senior Living’s provider-of-choice commitments across the assigned territory.

• Visit senior living communities regularly—up to five days per week—to understand needs, address concerns, and support service excellence.

• Coordinate communication and follow-through across community, operational, and clinical teams.

• Represent BAYADA professionally and reinforce a culture centered on compassion, excellence, and reliability.

• Organize competing priorities, manage follow-up, and ensure commitments are completed on time.

• Participate in an initial 90-day orientation that includes clinical and senior living orientation.

What you’ll bring

Required

• An active clinical license in good standing.

• Bachelor's degree.

• Strong organizational, communication, and relationship-building skills.

• A reliable, self-directed approach and the ability to manage work across multiple locations.

• A valid driver’s license and ability to travel throughout the assigned territory.

Preferred

• Experience in Home Health, senior living, outpatient care, or a related healthcare environment.

• Professional background as a PT, PTA, OT, COTA, SLP, RN, LPN, or another clinically licensed discipline.

• Experience building partnerships, supporting growth, or coordinating care across teams.

We welcome candidates from a range of clinical disciplines. Compensation will be based on experience, scope, and qualifications.

Work model and schedule

This is a field-based role with regular travel to senior living communities throughout Alexandria, Springfield, and Mount Vernon. The position may also require occasional visits to the Falls Church office. The standard schedule is Monday through Friday, 8:30 a.m. to 5:00 p.m. There is no on-call requirement.
