Job description

Oversee Homeless Outreach Team services that provide advocacy, service coordination, resource procurement, counseling, and support for clients enrolled in the program. Ensure that high-quality, high-fidelity Homeless Outreach services are provided to all clients. Responsible for hiring, supervising, and training Homeless Outreach personnel, as well as overseeing the overall management, evaluation, and budget of Homeless Outreach programs.

SPECIFIC RESPONSIBILITIES
  • Hire, supervise, and train staff members; conduct performance evaluations and initiate disciplinary actions as required.
  • Direct the day-to-day clinical operations of the team, including scheduling and ensuring adherence to agency policies. Lead daily morning meetings and weekly clinical review meetings to ensure clients' needs are addressed. Evaluate client status and ensure the intensity of services matches each client's needs.
  • Direct and coordinate the admission and discharge processes for clients entering and exiting the program. Assign clients to individual team members for primary responsibility. Monitor and coordinate referral, intake, and orientation processes for new clients.
  • Provide clinical supervision and conduct monthly individual supervision sessions with all staff.
  • Review the quality and quantity of services provided and documentation completed by staff.
  • Provide crisis intervention and support as needed.
  • Conduct regularly scheduled staff meetings to address operational and clinical concerns.
  • Ensure that documentation contained in client records meets accreditation and regulatory requirements.
  • Collaborate with the Core Service Agency and other community organizations to promote the agency's mission and secure community resources.
  • Communicate with community agencies regarding client concerns and needs; schedule and/or attend community meetings as required.
  • Ensure all client documentation is completed accurately, submitted in a timely manner, and complies with all applicable regulations and requirements.
  • Monitor medication management and health-related concerns for clients enrolled in the Homeless Outreach program.
  • Monitor the program budget, including billing, grant expenditures, and overall fiscal performance.
  • Develop expertise in the Homeless Outreach philosophy and maintain high fidelity to the Homeless Outreach Model of Care.
  • Ensure that the quality of services reflects the principles and standards of the Homeless Outreach Model of Care.
  • Ensure on-call coverage, including on-site response during evenings, nights, weekends, and holidays, as required.
  • Ensure the Homeless Outreach office operates efficiently and is clean, safe, and well maintained.
  • Develop departmental policies and procedures as needed.
  • Research, develop, write, edit, and submit compelling grant proposals, letters of inquiry, and supporting documentation.
  • Manage the grant lifecycle from prospect identification through award, implementation, reporting, compliance, and closeout.
  • Prepare and submit timely grant renewal applications to maintain existing funding sources.
  • Perform other duties as assigned.

EDUCATION, WORK EXPERIENCE AND JOB REQUIREMENTS

Education: Master’s degree in social work, Psychology, Counseling, Nursing, or a related field. Current Maryland licensure in a mental health profession is required. A minimum of three years of experience working with adults with serious and persistent mental illness is preferred.

Skill Requirement: One to six months of on-the-job training (OJT) is required for proficient job performance.

Job Requirements:

Excellent communication, problem-solving, and organizational skills.

Minimum of two years of supervisory experience.

Valid driver's license with a clean driving record.

Ability to work independently with minimal supervision.

COMMENTS

Must demonstrate knowledge of evidence-based mental health treatment practices and services for individuals experiencing homelessness.
